VCPR Document

Need an idea of how to write a VCPR? Looking for a ready-made version to adapt to your own needs? We've got you. We all know a VCPR is required for prescriptions & a written version is just best practice, but it can be challenging to get everything included that should be! This version of the VCPR is written for California, but this file includes both an editable Word document & a fillable PDF version that you can update to the needs & definitions of your state. I primarily use this document with school farms to lay out very specific manager & veterinarian expectations & responsibilities, but it can be adapted to any operation.

An editable Word document (which you can modify for your personal & state needs) & fillable PDF version of a CA-based VCPR document (the version I send to clients & can use for digital signatures)
Client information section, including spaces for primary & secondary contacts
Veterinarian of record (VOR) section, including space for an emergency contact if different
California state definition of what's required for a valid VCPR
Additional personalized requirements, highlighting appropriate medication use & my requirement of euthanasia training
Initial sites for the aforementioned conditions, for both producer & veterinarian
Signature area for both producer & veterinarian
